Case Management Services

Post Adoption Case Management and Service Planning

All families receiving post adoption and post permanency support services through Arms Wide will have an individual caseworker. Arms Wide Greater Houston and South Texas caseworkers are responsible for verifying adoption program eligibility and developing an individualized service plan based on both the adopted child’s needs and those of the family members as they relate to the child’s presence in the home.

The first phase of case planning begins when the caseworker first meets with each family to establish a personalized service plan. The plans are then reviewed and updated, preferably in face to face meetings with each adoptive family every six months, or more frequently if needed. Due to the complex nature of these particular post adoption services, Arms Wide’s experienced caseworkers build solid, long-term relationships with the adoptive families and support them to ensure the success of both the placement and overall well-being of their families.


Case management services are available 24 hours a day, 7 days a week. They include the following:

  • Bilingual services and support systems offered in English and Spanish
  • Education regarding permanency for youth issues and topics
  • Counseling with families about available support groups and other post adoption services
  • Client referrals to appropriate community resources
  • Participation in meetings with other agencies working with the adopted child or family
  • Aiding families in seeking third-party financial resources

Arms Wide provides post adoption and post permanency case management services in the Greater Houston and South Texas areas. The counties served by Arms Wide’s Post Adoption and Post Permanency Programs include:


Greater Houston Area –

Austin, Brazoria, Chambers, Colorado, Fort Bend, Galveston, Harris, Liberty, Matagorda, Montgomery, Walker, Waller, or Wharton County

South Texas Area –

Aransas, Bee, Brooks, Cameron, Duval, Hidalgo, Jim Hogg, Jim Wells, Kenedy, Kleberg, Live Oak, McMullen, Nueces, Refugio, San Patricio, Starr, Webb, Willacy, or Zapata County
